Thank you for choosing to work with Elizabeth Miller Hypnotherapy. This document explains the nature of hypnotherapy services in California, your rights as a client, and the permissions needed for recording your session. Please read carefully before signing.
1. Understanding the Nature of Hypnotherapy
Hypnotherapy is not a licensed profession in the State of California. California’s Business and Professions Code §2908 allows the use of hypnotic techniques for self‑improvement, vocational or avocational goals, or when working under referral from a licensed medical or mental‑health professional.
I am a Clinical Hypnotherapist, not a physician, psychologist, or other licensed medical provider. My services are considered complementary and alternative. They are not intended to diagnose, prescribe, or treat any medical or mental health condition.
If you require medical or psychiatric care, please contact a licensed professional or call 911 in an emergency. Referrals to licensed providers in San Diego are available upon request.
2. Scope of Services
Hypnotherapy supports self‑improvement, insight, and positive change. It is not psychotherapy. Your success depends on your willingness to participate, and you are responsible for your own interpretations and decisions that arise from your session.
Each session is unique to the individual. Elizabeth Miller Hypnotherapy does not accept responsibility for a person’s subjective experience in hypnosis.
3. Required Disclosures Under California SB 577
California law requires complementary and alternative practitioners to provide the following information in writing:
I am not a licensed physician.
My services are alternative or complementary to healing arts services licensed by the state.
The services I provide are not licensed by the State of California.
You will receive information about the nature of the services, the theory behind them, and my training and qualifications.
You will sign an acknowledgment confirming you have received this information.
A copy of your signed acknowledgment will be provided to you and on file for three years.
4. Recording Consent
Your session may be audio‑ or video‑recorded so you can revisit the material afterward, and so I can review the session to support your process with accuracy and care. Recording only happens with your clear, written permission. If you prefer not to be recorded, your session will unfold with the same depth and attention.
With your permission, portions of your session may also be used in future educational or creative projects, such as books, articles, or teaching materials. If any material is used, all identifying details will be removed. Your name, voice, image, and personal information will never be shared without your separate, explicit consent. You may withdraw permission at any time.
5. Cancellation Policy
To cancel or reschedule, you agree to provide at least 24 hours’ notice.
If you cancel with less than 24 hours’ notice, you are responsible for the full session fee.
If you reschedule instead of canceling, you will not be charged.
If you cancel the rescheduled appointment, you will forfeit the full session fee.
6. Client Responsibilities and Agreements
By signing this agreement, you acknowledge:
You understand that Elizabeth Miller is not diagnosing, prescribing, or treating any physical or mental condition.
You take full responsibility for any conclusions you draw from your session.
You release Elizabeth Miller from any liability related to your hypnotherapy session.
You understand that video/audio recordings provided to you are for personal use and must not be listened to while driving or performing any activity requiring full attention.
7. Acknowledgment and Consent
By signing below, you confirm that:
You have read and understand this Client Agreement, Disclaimer, and Recording Consent.
You understand the nature and scope of hypnotherapy services.
You understand your rights and responsibilities as a client.
You consent to receiving hypnotherapy services from Elizabeth Miller, M.Ed., CCHt.
